Il ne s\u2019agissait pas d\u2019une migraine pulsatile, ni d\u2019une douleur continue comme dans la c\u00e9phal\u00e9e de tension. Ce qui frappait, c\u2019\u00e9tait un \u00e9l\u00e9ment tr\u00e8s pr\u00e9cis : le mal de <strong>t\u00eate apparaissait ou s\u2019aggravait nettement en position debout ou assise<\/strong>, et s\u2019att\u00e9nuait d\u00e8s<strong> qu\u2019ils s\u2019allongeaient<\/strong>.  <\/p><p>Ce simple d\u00e9tail postural, en apparence secondaire, est en r\u00e9alit\u00e9 un <strong>signe clinique important<\/strong>, souvent n\u00e9glig\u00e9. Beaucoup de ces patients avaient d\u00e9j\u00e0 consult\u00e9 divers sp\u00e9cialistes, essay\u00e9 des m\u00e9dicaments, pass\u00e9 des examens neurologiques, sans jamais obtenir de diagnostic pr\u00e9cis. La c\u00e9phal\u00e9e orthostatique, lorsqu\u2019elle n\u2019est pas associ\u00e9e \u00e0 des causes \u00e9videntes comme une hypotension du liquide c\u00e9phalo-rachidien, a tendance \u00e0 \u00eatre class\u00e9e comme \u00ab non organique \u00bb. Pourtant, en y regardant de plus pr\u00e8s, il existe un lien possible avec la <strong>CCSVI<\/strong> qui m\u00e9rite d\u2019\u00eatre explor\u00e9.   <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-421c5bb elementor-widget elementor-widget-themeht-heading\" data-id=\"421c5bb\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">Qu\u2019est-ce que la c\u00e9phal\u00e9e orthostatique\u00a0?\n\n<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-babbd36 elementor-widget elementor-widget-text-editor\" data-id=\"babbd36\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>La c\u00e9phal\u00e9e orthostatique est un type de mal de t\u00eate qui appara\u00eet ou s\u2019intensifie lorsque l\u2019on se met debout. Elle est plus fr\u00e9quente dans les situations o\u00f9 le <strong>liquide c\u00e9phalo-rachidien (LCR)<\/strong> est perdu ou absorb\u00e9 trop rapidement, comme apr\u00e8s une ponction lombaire, un traumatisme, ou dans certaines maladies rares telles que <strong>l\u2019hypotension intracr\u00e2nienne spontan\u00e9e<\/strong>. <\/p><p>Cependant, il existe un groupe de patients pr\u00e9sentant les m\u00eames sympt\u00f4mes <strong>sans aucune anomalie document\u00e9e du LCR<\/strong>. Pas de fuite, ni de malformation. Que se passe-t-il alors ? C\u2019est dans ces cas que j\u2019ai commenc\u00e9 \u00e0 remarquer une co\u00efncidence importante : beaucoup d\u2019entre eux pr\u00e9sentaient des <strong>signes d\u2019alt\u00e9ration du drainage veineux c\u00e9r\u00e9bral et cervical<\/strong>, compatibles avec un tableau d\u2019<strong>insuffisance veineuse chronique c\u00e9r\u00e9brospinale<\/strong>, c\u2019est-\u00e0-dire la CCSVI.   <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c83b7b4 elementor-widget elementor-widget-themeht-heading\" data-id=\"c83b7b4\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">La CCSVI\u00a0: quand les veines entravent le retour veineux c\u00e9r\u00e9bral<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-d55cea8 elementor-widget elementor-widget-text-editor\" data-id=\"d55cea8\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>La CCSVI est une condition dans laquelle le sang a du mal \u00e0 s\u2019\u00e9couler correctement du cerveau vers le c\u0153ur, en raison de <strong>r\u00e9tr\u00e9cissements, compressions ou reflux anormaux<\/strong> dans les principales veines impliqu\u00e9es dans le drainage c\u00e9r\u00e9bral : les <strong>jugulaires internes, les veines vert\u00e9brales et la veine azygos<\/strong>.<\/p><p>En temps normal, le sang veineux devrait s\u2019\u00e9couler de fa\u00e7on libre et continue. Lorsque cet \u00e9quilibre est rompu, <strong>la pression veineuse intracr\u00e2nienne peut augmenter<\/strong> \u2014 surtout en position debout \u2014 car les veines de drainage sont partiellement ou totalement effondr\u00e9es. <\/p><p>C\u2019est pr\u00e9cis\u00e9ment cette stagnation qui peut provoquer la <strong>c\u00e9phal\u00e9e orthostatique<\/strong> : le sang veineux reste \u00ab pi\u00e9g\u00e9 \u00bb dans le cr\u00e2ne, g\u00e9n\u00e9rant une pression qui stimule les r\u00e9cepteurs de la douleur des m\u00e9ninges et des structures vasculaires c\u00e9r\u00e9brales. Ce m\u00e9canisme peut \u00e9galement expliquer d\u2019<strong>autres sympt\u00f4mes associ\u00e9s<\/strong> tels que le brouillard c\u00e9r\u00e9bral, les vertiges, les difficult\u00e9s de concentration ou les acouph\u00e8nes. <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-fce6989 elementor-widget elementor-widget-themeht-heading\" data-id=\"fce6989\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">Comment diff\u00e9rencier la c\u00e9phal\u00e9e li\u00e9e \u00e0 la CCSVI<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-3ba3f31 elementor-widget elementor-widget-text-editor\" data-id=\"3ba3f31\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Au fil du temps, j\u2019ai appris \u00e0 reconna\u00eetre certaines caract\u00e9ristiques cliniques typiques :<\/p><ul><li>La <strong>posture est d\u00e9terminante<\/strong> : les patients ressentent une aggravation en position debout et un soulagement d\u00e8s qu\u2019ils s\u2019allongent, parfois en quelques minutes \u00e0 peine.<\/li><li>La douleur peut \u00eatre <strong>diffuse, mais tend \u00e0 se localiser<\/strong> dans la zone occipitale, cervicale ou r\u00e9tro-orbitaire.<\/li><li>Elle s\u2019accompagne souvent d\u2019une sensation de <strong>pression dans les oreilles<\/strong>, de photophobie, de naus\u00e9es ou d\u2019une impression de \u00ab t\u00eate pleine \u00bb.<\/li><li>Certains patients signalent que m\u00eame le port d\u2019un <strong>collier cervical<\/strong> ou une <strong>l\u00e9g\u00e8re traction<\/strong> du cou r\u00e9duisent temporairement la douleur.<\/li><\/ul><p>Un point important : ces patients <strong>ne r\u00e9pondent pas aux traitements classiques<\/strong> de la migraine ou de la c\u00e9phal\u00e9e de tension. Beaucoup ont essay\u00e9 les triptans, les AINS, les antid\u00e9presseurs, les anti\u00e9pileptiques \u2014 sans am\u00e9lioration significative. Cela devrait d\u00e9j\u00e0 faire suspecter une cause \u00ab non classique \u00bb.  <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-ae72635 elementor-widget elementor-widget-themeht-heading\" data-id=\"ae72635\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">Les \u00e9tudes qui appuient ce lien<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-4d42a14 elementor-widget elementor-widget-text-editor\" data-id=\"4d42a14\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Le lien entre CCSVI et c\u00e9phal\u00e9e n\u2019est pas qu\u2019une simple observation clinique, il est \u00e9galement confirm\u00e9 par la litt\u00e9rature scientifique. Zamboni et d\u2019autres chercheurs ont \u00e9mis l\u2019hypoth\u00e8se qu\u2019une <strong>stase veineuse chronique<\/strong> pourrait g\u00e9n\u00e9rer une <strong>micro-hypoxie<\/strong>, un stress oxydatif et une <strong>neuroinflammation<\/strong>, autant de facteurs connus pour d\u00e9clencher et entretenir la perception de la douleur. <\/p><p>Certaines \u00e9tudes ont rapport\u00e9 une <strong>am\u00e9lioration de la c\u00e9phal\u00e9e orthostatique apr\u00e8s un traitement veineux<\/strong>, comme l\u2019angioplastie ou la d\u00e9compression chirurgicale en cas de compression extrins\u00e8que (par exemple, le syndrome d\u2019Eagle ou un conflit musculaire). Chez certains patients, une simple am\u00e9lioration de la posture nocturne ou l\u2019utilisation d\u2019oreillers cervicaux a permis une r\u00e9duction significative de la fr\u00e9quence et de l\u2019intensit\u00e9 de la douleur. <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b6766f8 elementor-widget elementor-widget-themeht-heading\" data-id=\"b6766f8\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">Pourquoi elle est souvent non diagnostiqu\u00e9e<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-7cfd29e elementor-widget elementor-widget-text-editor\" data-id=\"7cfd29e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Le grand obstacle est que la c\u00e9phal\u00e9e li\u00e9e \u00e0 la CCSVI <strong>n\u2019est pas encore reconnue officiellement dans les classifications neurologiques internationales<\/strong>. Elle est donc souvent \u00e9tiquet\u00e9e comme \u00ab c\u00e9phal\u00e9e non sp\u00e9cifique \u00bb ou, pire, \u00ab psychog\u00e8ne \u00bb. <\/p><p>Le manque de connaissances du personnel m\u00e9dical non sp\u00e9cialis\u00e9 dans les troubles vasculaires cr\u00e2nio-cervicaux fait que <strong>des examens cibl\u00e9s ne sont pas demand\u00e9s<\/strong>. Pourtant, une simple <strong>\u00e9chographie Doppler veineuse<\/strong>, pratiqu\u00e9e en position debout et allong\u00e9e, peut d\u00e9j\u00e0 fournir des indices tr\u00e8s clairs. <\/p><p>Lorsque le soup\u00e7on clinique est fort et que le patient pr\u00e9sente des sympt\u00f4mes associ\u00e9s (vision trouble, vertiges, troubles cognitifs l\u00e9gers), je recommande des <strong>examens plus approfondis<\/strong>, comme l\u2019IRM veineuse (MRV), le CBCT ou une phl\u00e9bographie s\u00e9lective.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element peut \u00eatre envisag\u00e9e dans des centres sp\u00e9cialis\u00e9s.<\/li><li>En pr\u00e9sence de <strong>compressions m\u00e9caniques<\/strong>, comme dans le syndrome d\u2019Eagle, une intervention chirurgicale est possible.<\/li><li>Dans les cas moins graves ou en attente d\u2019intervention, un <strong>traitement postural, une th\u00e9rapie manuelle cibl\u00e9e<\/strong>, l\u2019utilisation de colliers orthop\u00e9diques et des changements de mode de vie peuvent aider \u00e0 g\u00e9rer les sympt\u00f4mes.<\/li><\/ul><p>L\u2019essentiel est de <strong>ne pas n\u00e9gliger<\/strong> la composante vasculaire, car chez de nombreux patients, c\u2019est justement la cl\u00e9 pour expliquer des sympt\u00f4mes autrement \u00ab incompr\u00e9hensibles
