In recent years, my clinical work has led me to explore a possible connection that, while still on the fringes of mainstream scientific discourse, deserves a closer and more thoughtful examination: the role of chronic cerebrospinal venous insufficiency (CCSVI) in two complex and debilitating syndromes\u2014fibromyalgia and chronic fatigue syndrome, also known as CFS or ME.
These are undoubtedly multifactorial conditions. However, an increasing number of patients affected by them present with clinical signs suggestive of impaired cerebral venous drainage. In this article, I aim to share my clinical experience, compare it with the available scientific literature, and offer a perspective that moves beyond the purely neurological paradigm. <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

What is CCSVI and Why It Might Matter<\/h2> \r\n\r\n <\/div>\r\n\r\n \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

CCSVI is a condition in which venous return from the brain to the heart is impaired\u2014typically due to stenosis, extraluminal compression, or pathological reflux in the internal jugular veins, vertebral veins, or the azygos vein. This results in chronic venous congestion which, over time, may lead to increased intracranial pressure, tissue hypoxia, and a state of low-grade chronic inflammation. <\/p>

Despite the controversies that arose following its association with multiple sclerosis, I have observed that many patients diagnosed with fibromyalgia and CFS exhibit symptoms that closely align with the clinical picture of CCSVI: positional headaches, a sensation of intracranial pressure, visual disturbances, tinnitus, chronic fatigue, and\u2014most notably\u2014a subjective improvement of symptoms when lying down.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

\n\t\t\t\t\t\t\t\t\t

Fibromyalgia and CFS\/ME are syndromes that frequently overlap. Fibromyalgia is marked by widespread musculoskeletal pain, fatigue, and sleep disturbances, while CFS is characterized by profound exhaustion that is not relieved by rest, brain fog, and intolerance to physical or mental exertion\u2014commonly referred to as post-exertional malaise. Clinically, I have observed that many patients with these diagnoses also exhibit significant autonomic dysfunctions, including postural tachycardia, orthostatic intolerance, vasovagal episodes, and blood pressure fluctuations, all of which point toward autonomic nervous system involvement. <\/p>

What strikes me most, however, is the frequent presence of positional symptoms: patients who feel worse when standing, experience relief when lying down, and report a gradual buildup of pressure in the head as the day progresses. These are all signs that lead me to suspect a vascular component\u2014specifically, an issue with venous drainage. <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

The Scientific Evidence So Far: A Landscape in Progress<\/h2> \r\n\r\n <\/div>\r\n\r\n \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

\n\t\t\t\t\t\t\t\t\t

Although few studies have directly examined the relationship between CCSVI and these two syndromes, several lines of research have documented abnormalities in cerebral blood flow<\/strong> and vascular regulation<\/strong> in patients with fibromyalgia or CFS. While these findings do not specifically reference CCSVI, they align closely with the hypothesis that venous dysfunction may act as a contributing factor\u2014or even an amplifier\u2014of symptoms. <\/p>

A 2011 study conducted by Ford et al.<\/a><\/strong><\/span>, using perfusion MRI, found reduced blood flow to the thalamus in fibromyalgia patients, along with functional disconnection between pain-related and cognitive brain regions. What struck me is that similar patterns are also observed in patients with CCSVI, suggesting that venous hypoxia may play a role in pain regulation. <\/p>

Montoro et al.<\/strong><\/span><\/a>, using transcranial Doppler, observed reduced variability in cerebral blood flow among individuals with fibromyalgia\u2014an observation I have often confirmed myself through dynamic testing in upright posture. This rigidity in flow regulation is clinically associated with worsening fatigue, increased pain, and reduced exercise tolerance. <\/p>

In another publication in PLOS ONE<\/em>, Rodr\u00edguez and colleagues described increased complexity in resting-state brain signals in individuals with fibromyalgia\u2014an indicator of instability in vascular regulation. While CCSVI is not mentioned, the similarities with the symptoms observed in my CCSVI-positive patients are striking. <\/p>

Finally, a study by Kaya et al. showed that flow volumes in the carotid and vertebral arteries correlate with symptom severity, even if absolute values are not necessarily abnormal. This is a crucial insight: it\u2019s not always the amount of blood flow that\u2019s impaired, but rather how that flow is regulated and how it responds to the brain\u2019s physiological demands.<\/strong> <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

CCSVI as an Amplifier of Dysfunction<\/h2> \r\n\r\n <\/div>\r\n\r\n \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Based on years of clinical experience, my hypothesis is that CCSVI may act as an aggravating factor<\/strong>\u2014or even a triggering co-factor<\/strong>\u2014in individuals who are already predisposed. Not all patients with fibromyalgia or CFS have CCSVI, but many show signs of impaired venous outflow, particularly when structural anomalies are present, such as compression from elongated styloid processes or scalene muscles, or in cases of cervical instability. <\/p>

Inefficient venous drainage can worsen intracranial congestion, autonomic dysfunction, chronic fatigue, and even pain sensitivity. This is why it\u2019s essential to include dynamic venous color Doppler ultrasound in the evaluation of these patients<\/strong>\u2014to identify any mechanical or functional obstacles to blood returning from the brain. <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Lessons from the Multiple Sclerosis Experience<\/h2> \r\n\r\n <\/div>\r\n\r\n \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Although this article does not focus specifically on multiple sclerosis, it’s worth noting that a study by Zamboni et al. reported a significant reduction in fatigue among MS patients following endovascular treatment for CCSVI. While the underlying mechanisms may differ, chronic fatigue is a shared symptom across all three conditions, and the improvement observed in that context supports the idea that chronic venous congestion can have a direct impact on systemic symptoms<\/strong>. <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Practical Implications and Future Directions<\/h2> \r\n\r\n <\/div>\r\n\r\n \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Medicine cannot ignore clinical evidence simply because large-scale randomized trials are lacking. It\u2019s true that CCSVI is not yet universally recognized by the neurological community, but the data gathered so far\u2014combined with real-world clinical observations\u2014call for a reassessment. Patients with fibromyalgia and CFS who show clinical signs consistent with impaired venous drainage deserve to be investigated, not dismissed out of hand. <\/p>

The future calls for multicenter studies, standardized diagnostics, advanced imaging, and an integrated approach involving neurology, angiology, and functional medicine. But in the meantime, there is already much we can\u2014and must\u2014do. <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t
