His condition progressively worsened and proved resistant to conventional treatments.<br>It was only after a targeted <strong>venous decompression<\/strong>\u2014without any PTA\u2014that his symptoms completely disappeared. <\/p><p>A case that challenges the traditional approach to CCSVI and paves the way for a broader, more dynamic, and personalized diagnostic perspective.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-3b4c88f elementor-widget elementor-widget-themeht-heading\" data-id=\"3b4c88f\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">The clinical case: when the neck blocks the brain\n<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-8d0be68 elementor-widget elementor-widget-text-editor\" data-id=\"8d0be68\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>The patient\u2014a 30-year-old man\u2014presented with:<\/p><ul><li>Orthostatic headache (worsening when upright, improving when lying down)<\/li><li>Pulsatile tinnitus<\/li><li>Blurred vision<\/li><li>Debilitating brain fog<\/li><li>Marked decline in work capacity, reduced to 50%<\/li><\/ul><p>After numerous tests and treatment attempts\u2014including a blood patch for a suspected CSF leak\u2014a <strong>venography with dynamic maneuvers<\/strong> was finally performed. The imaging revealed a <strong>mechanical compression of the right internal jugular vein<\/strong>, pinched between the styloid process and the transverse process of C1.<\/p><p>Head rotation worsened the venous outflow, triggering cerebral congestion and the activation of collateral circulation. Cerebral venous manometry revealed a pathological <strong>pressure gradient of 10\u202fmmHg between the torcular Herophili and the right atrium<\/strong>. <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-8ed7669 elementor-widget elementor-widget-themeht-heading\" data-id=\"8ed7669\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">The treatment: no balloons, only targeted decompression\n<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-0c82592 elementor-widget elementor-widget-text-editor\" data-id=\"0c82592\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Unlike the classic CCSVI model focused on PTA, the effective treatment in this case was <strong>surgical<\/strong>: the patient underwent a <strong>C1 tuberculectomy<\/strong>\u2014the removal of the C1 tubercle that was compressing the jugular vein.<\/p><p>Clinical and instrumental outcomes:<\/p><ul><li>Intracranial pressure at the torcular dropped from 16 to 7 mmHg<\/li><li>Brain-to-heart pressure gradient reduced from 10 to 4 mmHg<\/li><li>Complete resolution of brain fog, headache, tinnitus, and fatigue<\/li><li>Full return to work<\/li><\/ul><p>This is a key point: <strong>no PTA was needed<\/strong>. The issue wasn\u2019t a faulty valve\u2014it was a <strong>bony compression<\/strong>, just like the ones described on my dedicated page about <span style=\"text-decoration: underline;\"><strong><a href=\"https:\/\/www.raffaellopagani-ccsvi.it\/en\/department\/extrinsic-compressions-and-ccsvi\/\">extrinsic compressions and CCSVI<\/a><\/strong><\/span>. <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-de44c44 elementor-widget elementor-widget-themeht-heading\" data-id=\"de44c44\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">Pathophysiological basis: why venous congestion disrupts the brain\n<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-552dc32 elementor-widget elementor-widget-text-editor\" data-id=\"552dc32\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>The study outlines a condition known as <strong>cerebral venous congestion (CVC)<\/strong>\u2014a state in which venous blood fails to drain properly from the brain. This leads to stasis, elevated intracranial venous pressure, and, as a result, functional and metabolic dysfunction.<\/p><p>In animal models, jugular vein ligation has been shown to cause:<\/p><ul><li>Microglial activation (neuroinflammation)<\/li><li>Blood\u2013brain barrier dysfunction<\/li><li>Impaired memory and cognitive function<\/li><li>Gait disturbances and behavioral changes<\/li><\/ul><p>This supports the idea that venous congestion is not just a passive hemodynamic delay, but an <strong>active mechanism of cerebral injury<\/strong>.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-88e6576 elementor-widget elementor-widget-themeht-heading\" data-id=\"88e6576\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">A diagnosis to rethink: looking only at the lower neck isn\u2019t enough\n<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-35f0475 elementor-widget elementor-widget-text-editor\" data-id=\"35f0475\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>The CCSVI model introduced in 2009 focused primarily on <strong>malformed valves<\/strong> in the lower segment of the internal jugular veins, treated with PTA. But today, we know that: <\/p><ul><li>The most <strong>common compressions occur higher up<\/strong>, often at the base of the skull<\/li><li>The <strong>affected segments are dynamic<\/strong>\u2014compression happens only in certain head positions<\/li><li><strong>Standard diagnostic tests may come back negative<\/strong> if they don\u2019t include dynamic maneuvers and pressure measurements<\/li><\/ul><p>That\u2019s why, in my clinical practice, I include:<\/p><ul><li>CT venography with provocative maneuvers<\/li><li>Dynamic assessment during flexion, rotation, and tilt<\/li><li>Measurement of the brain-to-heart pressure gradient (BHG)<\/li><li>Detailed analysis of styloid and C1 morphology, as well as surrounding ligaments and neck muscles<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e1779f9 elementor-widget elementor-widget-themeht-heading\" data-id=\"e1779f9\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">Differential diagnosis: when headache hides venous obstruction\n<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-8b6a4df elementor-widget elementor-widget-text-editor\" data-id=\"8b6a4df\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>In the case described, the headache was orthostatic\u2014worsening when upright and improving when lying down. This pattern can easily be mistaken for: <\/p><ul><li>Cerebrospinal fluid (CSF) hypotension<\/li><li>Vestibular migraine<\/li><li>Idiopathic intracranial hypertension<\/li><\/ul><p>However, the presence of <strong>brain fog, pulsatile tinnitus, symptom worsening with head rotation<\/strong>, and <strong>normal CSF pressure<\/strong> all point toward a venous disorder.<br>This is exactly why <strong>it\u2019s crucial not to rely solely on brain MRI<\/strong>\u2014it may miss the root cause entirely. <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b94f87e elementor-widget elementor-widget-themeht-heading\" data-id=\"b94f87e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">Every patient has their obstacle, every obstacle its therapy\n<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-85ab0c4 elementor-widget elementor-widget-text-editor\" data-id=\"85ab0c4\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>This case confirms what I\u2019ve observed for years in clinical practice: there is no one-size-fits-all solution for CCSVI. The idea of a single treatment for every patient is fundamentally flawed.<br>What\u2019s needed instead is a tailored approach, one that takes into account:  <\/p><ul><li>The type of obstruction (valvular abnormality, bony compression, intrathoracic or intracranial stenosis)<\/li><li>Its location and hemodynamic impact<\/li><li>The patient\u2019s specific clinical symptoms<\/li><\/ul><p>PTA can be helpful in certain scenarios, but it\u2019s rarely effective when high or dynamic compressions are involved. In such cases, targeted <strong>surgery is often the only truly effective option<\/strong>. <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-3e3485c elementor-widget elementor-widget-themeht-heading\" data-id=\"3e3485c\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">The key message: brain fog is real and often treatable\n<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-21069a0 elementor-widget elementor-widget-text-editor\" data-id=\"21069a0\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Too often, people suffering from brain fog, mental fatigue, and lack of focus are told it\u2019s all \u201cjust psychological.\u201d But this study shows that there can be real, visible, and <strong>treatable venous obstructions behind these symptoms<\/strong>. <\/p><p>Cerebral venous congestion is real, it causes symptoms, and it can be measured.<br>The brain-to-heart pressure gradient (BHG) is one of the most underrated tools\u2014yet it\u2019s often the key to identifying who has the potential to improve. <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-2f742af elementor-widget elementor-widget-themeht-heading\" data-id=\"2f742af\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">An invitation to personalized evaluation<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-f34658f elementor-widget elementor-widget-text-editor\" data-id=\"f34658f\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>The 2022 article published in <em>Neurology<\/em>, is a turning point.
