And sometimes, all it takes is shifting from lying down to sitting up to trigger a cascade of symptoms.   <\/p><p>I&#8217;ve been working with <a title=\"What is CCSVI?\" href=\"https:\/\/www.raffaellopagani-ccsvi.it\/en\/department\/ccsvi-cerebrospinal-venous-insufficiency\/\" target=\"_blank\" rel=\"noopener\"><span style=\"text-decoration: underline;\"><strong>CCSVI<\/strong><\/span><\/a> for years\u2014a condition that&#8217;s still relatively unknown, yet can have a very real impact on quality of life. And the more I get to know patients, the more I realize just how much posture\u2014yes, something as simple as how we sit, sleep, or hold our head\u2014can affect overall well-being, especially in people dealing with venous drainage issues. <\/p><p>If you suffer from unexplained headaches, a sense of pressure in your head, blurred vision, or ringing in your ears that gets worse when you change position, you may have already felt this connection in your own body\u2014whether you realized it or not.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-5548d5a elementor-widget elementor-widget-themeht-heading\" data-id=\"5548d5a\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">What is venous flow and why does it matter?\n<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-491fd0b elementor-widget elementor-widget-text-editor\" data-id=\"491fd0b\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Venous flow is the system that allows &#8220;used&#8221; blood to return to the heart, where it can be reoxygenated in the lungs. In the brain, this process happens primarily through the internal jugular veins. <\/p><p>Unlike arteries, veins don\u2019t benefit from the heart\u2019s direct pumping action.<br>Their flow depends on: <\/p><ul><li>Gravity<\/li><li>Intracranial pressure<\/li><li>Body position<\/li><li>Presence of obstructions or compressions<\/li><\/ul><p>When venous drainage isn\u2019t efficient, it can lead to intracranial congestion\u2014bringing on symptoms that range from headaches and pressure behind the eyes to visual disturbances and cognitive issues.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-2c88432 elementor-widget elementor-widget-themeht-heading\" data-id=\"2c88432\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">CCSVI: when drainage gets blocked\n<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-2f96d64 elementor-widget elementor-widget-text-editor\" data-id=\"2f96d64\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>CCSVI (Chronic Cerebrospinal Venous Insufficiency) involves narrowings, obstructions, or malformations in the main venous outflow pathways of the brain and spinal cord. The causes may include: <\/p><ul><li>Internal jugular vein stenosis<\/li><li>Compression from muscles or bony structures<\/li><li>Malformed venous valves<\/li><li>Abnormal pressure in the dural venous sinuses<\/li><\/ul><p><strong>Common symptoms include:<\/strong><\/p><ul><li>Positional headaches<\/li><li>Head pressure<\/li><li>Pulsatile tinnitus<\/li><li>Intermittent visual disturbances<\/li><li>A \u201cheavy head\u201d sensation<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-f76bd09 elementor-widget elementor-widget-themeht-heading\" data-id=\"f76bd09\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">Does posture change the flow?\n<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-61849f8 elementor-widget elementor-widget-text-editor\" data-id=\"61849f8\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Oh yes- it absolutely does.<\/p><p>When <strong>lying down<\/strong>, the jugular veins dilate and drain blood efficiently.<\/p><p>But when <strong>standing<\/strong>, these veins naturally tend to collapse. At that point, drainage shifts to secondary pathways, like the vertebral plexus. <\/p><p>But what if there are already obstructions? <\/p><p>A few examples:<\/p><ul><li>Sleeping with your neck hyperextended can reduce blood flow<\/li><li>Keeping your head turned to one side may compress a jugular vein<\/li><li>Some people find relief only in specific positions\u2014like lying on one side or with their head elevated<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-9d51c71 elementor-widget elementor-widget-themeht-heading\" data-id=\"9d51c71\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h3 class=\" \">When gravity works against you\n<\/h3>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-68a2d00 elementor-widget elementor-widget-text-editor\" data-id=\"68a2d00\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>In theory, gravity should help blood drain from the head. But if there are bottlenecks\u2014like stenosis, muscle compressions, or stuck valves\u2014it turns into an obstacle. Blood starts to pool, pressure rises, and symptoms get worse.  <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-eb08896 elementor-widget elementor-widget-themeht-heading\" data-id=\"eb08896\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">What science tells us\n<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b5d5e5e elementor-widget elementor-widget-text-editor\" data-id=\"b5d5e5e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>In recent years, several scientific studies have highlighted how <strong>cerebral venous flow<\/strong> can be significantly affected by body posture, particularly in patients with <strong>CCSVI (Chronic Cerebrospinal Venous Insufficiency)<\/strong>. Let\u2019s take a closer look at some key findings. <\/p><p><strong>Study by Prof. Paolo Zamboni et al. \u2013 PLoS ONE (2009) <\/strong><br>Professor Paolo Zamboni\u2019s team\u2014among the first to systematically investigate CCSVI\u2014used EcoColor Doppler ultrasound in patients with multiple sclerosis to assess venous flow in the <strong>internal jugular veins<\/strong> and the azygos system.<br>One of the most striking findings was the <strong>abnormal flow behavior in the supine position<\/strong>:<br>In several cases, the <strong>jugular veins showed absent, blocked, or even reversed flow<\/strong>, meaning blood was flowing back toward the brain instead of draining normally toward the heart.<br>This is highly unusual\u2014lying down should cause the jugular veins to dilate and drain efficiently.<br>When that doesn&#8217;t happen, it strongly suggests the presence of <strong>obstructions, stenosis, or malformed valves<\/strong>, which may behave differently depending on gravity and head and neck position. <\/p><p><strong>Research by Haacke et al. \u2013 AJNR (2010) <\/strong><br>Dr. Haacke conducted extensive studies using <strong data-start=\"1519\" data-end=\"1542\">MRI and venous mapping techniques<\/strong> to evaluate changes in blood flow caused by <strong>external compressions<\/strong>.<br>His findings revealed that some patients experience <strong>mechanical interference<\/strong> from surrounding structures\u2014such as compression of the internal jugular veins by the <strong>scalene muscles<\/strong>, the <strong>omohyoid muscle<\/strong>, or bony structures like the styloid process and the <strong>hyoid bone<\/strong>.<br>What\u2019s particularly striking is that even <strong>slight head rotation or tilting can alter the hemodynamic profile<\/strong>\u2014sometimes improving flow, sometimes reducing it, or even causing it to cease entirely.<br>This suggests that everyday <strong>neck posture<\/strong>\u2014like sleeping with the head turned or using pillows that are too high\u2014may worsen venous congestion in individuals who are already predisposed.<br><br><br><br> <\/p><p><strong>Clinical Observations by Dr. Salvatore Sclafani \u2013 Understanding CCSVI<\/strong><br>Dr. Salvatore Sclafani, an expert in interventional radiology, has documented numerous cases where <strong>venous valve behavior appeared to be dynamic<\/strong>, meaning it changed depending on head and body position.<br>In some patients, venography revealed <strong>valves that looked completely normal while sitting upright<\/strong>\u2014but would <strong>shut closed when lying down<\/strong>, effectively blocking venous outflow.<br>In other cases, abnormalities only became apparent during specific maneuvers, such as neck rotation or forced extension, reinforcing the idea that extracranial <strong>venous flow is heavily influenced by movements of the cervical spine<\/strong>.<\/p><p>These studies\u2014though different in methodology\u2014point to a common conclusion:<\/p><p><strong>Posture, neck movement, and individual venous anatomy can significantly impact how blood drains from the brain.<\/strong><\/p><p>And in patients with CCSVI, these factors not only worsen symptoms, but can also <strong>mask or distort diagnostic results<\/strong>\u2014especially if exams are not performed dynamically or in upright\/seated positions.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e519630 elementor-widget elementor-widget-themeht-heading\" data-id=\"e519630\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"themeht-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\r\n        <div class=\"\">\r\n\r\n          <h2 class=\" \">Symptoms that shift with posture: an important clue\n<\/h2>       \r\n\r\n        <\/div>\r\n\r\n        \t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-6409c54 elementor-widget elementor-widget-text-editor\" data-id=\"6409c54\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Many CCSVI patients report:<\/p><ul><li>Headaches that worsen when standing<\/li><li>Pressure behind the eyes upon getting up<\/li><li>\u201cBrain fog\u201d and a sense of imbalance<\/li><li>Positional pulsatile tinnitus<\/li><li>A feeling of fluid that just won\u2019t drain<\/li><\/ul><p>These are not symptoms to ignore. A standard brain MRI, for example, is typically performed with the patient lying down\u2014it may appear completely normal, even in cases where the patient experiences clear symptoms while standing or sitting. This is one of the reasons why so many cases of CCSVI go undetected.  <\/p><p>Some specialized centers offer <strong>upright MRI<\/strong> (also known as weight-bearing or vertical MRI), which allows clinicians to observe changes in vascular and spinal structures while the patient is in an upright position. Though still not widely available, this technique can be extremely valuable in revealing compressions or flow abnormalities that are simply not visible when lying flat. <\/p><p>However, the most accessible and informative test remains the <strong>dynamic EchoColor Doppler<\/strong> of the neck veins. This ultrasound exam allows real-time observation of venous flow behavior during changes in position\u2014from lying down to sitting or standing.
