Intracranial hypertension (IH) occurs when the pressure inside the skull rises above normal levels, causing potentially serious neurological symptoms. Once often labeled as idiopathic or benign, current research now urges us to better understand the underlying mechanisms\u2014especially the role of the cerebrospinal venous system and its dysfunctions, known as Chronic Cerebrospinal Venous Insufficiency (CCSVI). <\/p>

Problems with venous drainage caused by narrowing or compression of the internal jugular veins and dural venous sinuses lead to blood pooling and increased pressure, which can cause or worsen intracranial hypertension.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

The brain is enclosed in a rigid cavity\u2014the skull\u2014and is surrounded by a clear fluid called cerebrospinal fluid (CSF), which cushions and protects the central nervous system. CSF is continuously produced by specialized structures called choroid plexuses. It circulates through the brain’s ventricles and around the brain and spinal cord, then is reabsorbed into the large dural venous sinuses via arachnoid granulations. <\/p>

The balance between CSF production and absorption maintains steady intracranial pressure. For CSF to be properly absorbed, intracranial pressure must be slightly higher than the venous pressure in the dural sinuses, creating a pressure gradient that allows CSF to flow into the venous bloodstream. <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

To better understand this balance, it\u2019s often compared to a sink: the faucet represents the steady production of CSF, and the drain corresponds to its absorption into the veins. If the drain becomes clogged or narrowed, water (CSF) builds up in the sink (the skull), causing the level\u2014and thus the pressure\u2014to rise. <\/p>

This analogy explains why an obstruction in the venous system, such as in CCSVI, can lead to increased intracranial pressure.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Normally, cerebral venous pressure is low, around 10-18 mmHg. In patients with idiopathic intracranial hypertension (IIH), venous pressure can rise significantly, sometimes exceeding 40 mmHg. <\/p>

Clinical studies show that CSF pressure directly correlates with venous pressure in the dural sinuses, particularly at the junction between the superior sagittal sinus and the transverse sinuses (the torcula). When venous pressure increases, CSF pressure and thus intracranial pressure rise proportionally. <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t
